New Patient Information

Due to high demand in the St George area, we are currently adding new patients from a waiting list. To be placed on the waiting list, submit your new patient paperwork on this website and our office staff will contact you to place you on the list. 


We’re glad you are considering making Southwest Internal Medicine your medical home.  We assure you we will do our best to help you maintain the healthy, active lifestyle that you desire, and provide your health care in a modern, efficient way.

Before Your First Appointment

Before your first appointment, please fill out your new patient information forms online or bring the completed forms with you.  Completing these forms beforehand will help us on your appointment day.  Also, make sure you have your insurance cards and a list of your current medications ready to bring to your appointment.

What To Expect When You Come In

When you come to our office, make sure you check in at the front desk.  Our helpful staff  can assist you with your new patient paperwork, if you have any questions.   We will copy your insurance cards,  verify your information and have you sign the privacy forms.

Depending on your insurance, if you have to pay a copay, we will collect it at this time.  We will also request to take your picture to put into your file  for identification.

After you check in, one of our medical assistants will call you back to take your height, weight, blood pressure, and to get important information about your visit.  Be sure you tell her  if you are taking any medications or need to update your medical history to include any changes.

After this information is recorded, the doctor will come in to consult with you, review your medical history, perform a physical examination, and address any medical problems or questions you might have.  Feel free to write down ahead of time any particular questions or concerns you want to discuss.

When you and the doctor are finished you will be escorted to the check out desk, where any followup appointments and referral appointments can be made.  Also, if the doctor has recommended you have xrays, mri’s etc. those appointments will be scheduled  for you.
